Admission Guidelines


Admission to our services requires a physician’s order and is based on the patient’s specific care needs, homebound status, and the types of services we can provide directly or coordinate with other trusted organizations.

Once admitted, we will develop a personalized care plan and work with you to set meaningful health goals. When those goals are achieved, you will be ready for discharge from our services.

Medicare Requirements & Payment Details
 

To qualify for Medicare coverage (and many private insurance plans) for home health care services, the following conditions must be met:

  • Homebound Status – Determined by your physician and home health care team. Being “homebound” means that leaving your home requires considerable effort or causes significant fatigue. Certain exceptions may apply, which you can discuss with your care provider.

  • Medical Need – You must have a recent illness, injury, or a worsening of an existing condition that requires skilled nursing on an intermittent basis and/or physical therapy or speech therapy. Once these needs are established, you may also qualify for additional services, such as occupational therapy, medical social work, or home health aide support.

Physician Visits

If your home health care is covered by Medicare, you must have had an in-person visit with your physician within the 90 days prior to starting services, or within 30 days after your care begins. Your physician must provide written certification confirming the visit occurred within this required timeframe. Additionally, your care must be physician-ordered and deemed medically necessary for insurance coverage to apply.

Charges & Payment Information
 

ValleyChoice Home Health Extenders accepts Medicare, Medicaid, private insurance plans, workers’ compensation insurance, and private pay. Before your care begins, we will inform you and/or your authorized representative of all applicable charges and payer sources.

We will bill your insurance directly for all covered services and accept Medicare-assigned payments for eligible clients. If services are ordered that are not covered by your insurance (including Medicare), we will notify you in advance and help you make any necessary arrangements for your care.

If you change insurance carriers or enroll in a Medicare Advantage Plan, HMO, or Hospice, please notify us immediately. We will review your continued eligibility for services and determine whether we can continue care under your new coverage. If we cannot, we will assist you in finding appropriate alternative care.
